James, it should also be remembered that qc is limited to visual. There just isn't any way possible to do anything more. It's been automated with cameras at this point. Neither priming compound nor powder charge could be absolutely proven. Run out by the millions there are bound to be lots of failures. We can't even make perfect jelly beans.

To a large extent automation has made products far more reliable, but automation also allows errors that might not get past hand work. I'd rather have an electronic camera checking every primer for an anvil.
 
I wouldn't send it back to Ruger. Give it a good cleaning, before shooting run a patch down the barrel to remove any oil , always a good thing to cleaned your new gun when you first bought it before shooting it . I also feel it was a weak case head . Next time out take your time make sure every round ejects & feeds properly . Good learning experience first time out . You could load up a magazine , finger OFF the trigger just hold the grip , to see how the rounds eject ,as a test , an pointed in a safe direction . I've seen heads ripe off rifle rounds , from reloading hot and sizing problems , never saw it on a 22 lr. round . Always a first time , your luck . Will probably never happen again. Anyway makes a good story to tell your friends . It's a nice gun , have fun shooting it . Be Safe.
