My state recently passed what they call "Constitutional Carry" so no permit is required, although my wife and I still have our permits and renew them every time. I also have the LEOSA permit card but don't carry it every day as it's more for out of state travel.
When I carry I always have one thing that you didn't put on your list and that is a reload. With semi autos it isn't about round count but clearing a malfunction. On the rare occasion when I carry a revolver it is primarily to re-charge the cylinder if any rounds were fired.
As far as the other stuff, I usually have a folding knife in one pocket or another but being an old geezer I just never got into the cell phone thing so it's always in the truck but I don't need to hear from anyone bad enough to carry the dang thing. (smile)
I haven't yet found the flashlight I can carry 24/7, . . . if I do, . . . I'll add it, . . . meantime, . . . CHL, holster, knife, mag (or 2 or 3), knife, cell phone, and an all steel 5" 1911.

--My beloved automatic knife (gasp) I know it's "illegal" but it's like a natural extension of my body. Even expensive folders feel clunky and slow compared to it.

Like you, I went a long time trying to find a pocketable flashlight. Finally found it in the sporting goods section at Walmart, of all places. Tiny little rectangular LED flashlight, about half the width of a credit card. Fits right on my keychain and I don't even know it's there. About three bucks, if I recall. Amazingly bright.

Evening - I carry a 5" Coleman LED Flashlight. (90 Lumens)....Purchased from WalMart - $17.97
I have 2 good flashlights. A very good keychain light I got at Target. It was an Eddie Bauer one. Sadly they no longer sell this light (tried to buy a couple more of them). Also have a really good single AAA LED light from Home Depot (Husky). Bright but not super bright so it has good battery life too (especially with the Energizer Li batteries). Really a good light and small. I bought a whole bunch of these. Never go anywhere without a flashlight and a pocket knife. And of course a wallet & cellphone always present these days (and a cell phone charger stays in the car).
As for carry (when I can carry), the gun, the holster (IWB), and the carry permit. The gun holds 16 rounds so no need for the extra mag.
I live in a small town in the mid west. Crime is really not an issue unless you go look for it. I usually carry in a pancake holster my well worn Colt Cobra,
Taurus M450Ti 45 Colt or my Ruger SA 3 3/4 barrel revolver in 45 colt. i have had a CCL for many many years. If I can't defend myself with 5 or 6 rounds I will retreat. with our lithigous society one has to careful not to loose everything to a lawyer.

Keys, Phone, Gun. In the winter when I have coats with lots of pockets on maybe I will carry some other stuff, but I dont like to have so much stuff in my pockets.

I never leave the house unarmed,My pistol(IWB) spare mag in my back pocket,Wallet,pocket clip Knife and thats about it.Flash light and head lamp stay in the truck,But I do have good night sights on all my carry guns.
If I am threatened, and I can abate the threat by demonstrating that I am armed,- I put a very high priority on calling 911 to report and describe the threat.
I feel it is a citizen's duty to give law enforecemnt a chance to apprehend the culprit before they can threaten/attack another innocent citizen.

My p3at lives in my pocket, I carry a m&pc,Pf9,or a 1911cs, a gerber clip knife and a little case peanut, A little led AAA light, a extra mag for the big gun. and a set of earplugs cause i like to shoot.
