Some people have pushed the envelope of what kind of pain they can endure, and, can fight through it. Women have an edge, since giving birth can be incredibly painful.
I guess most of the fighters I've been around would keep punching until they couldn't anymore. You can get really battered, but, very few points on the human body create so much pain, or damage that death or incapacitation occur.
Most COM hits are in that category, unless you hit spine or heart. Kidney punches ARE, for example...
