I shoot almost exclusively handloads. Some if my guns I've never shot any factory loads in. Not sure why reloads are blamed for gun trouble unless poor loads are used.
I shot SOMEBODY ELSES reloads and had a bullet creep forward, but not one of mine. Simply poor work on their part. I don't shoot anyone elses reloads anymore unless I know them real well and know how they load.
Crud under the extractor star? Couple times in centerfire guns they got sticky to work, but they did still work. This happens from not cleaning under the star. If I didn't do anything else to clean a DA revolver, I would clean under the extractor star. On rimfire DA's, a few bricks of cruddy ammo and they start to get gummy, but a simple toothbrushing under the star and they are back in business for another few bricks of ammo.
I have had the extractor rod come loose a couple times, they turn back in pretty easy, but simply torqueing them well with leather, copper or wood padded vise or plier jaws and a couple empties in the chambers will take care of this. Never had one come loose that was torqued well. They should be checked while handling and dry firing.
Never had a bent yoke, or a primer back out. How many rounds is "enough"? Been at this for about 30 years or so.
