Were millitary pistols carried cocked and locked?

I was trained as an MP in 1969 and we were instructed to carry hammer down on an empty chamber and 5 rounds in the magazine. I am assuming that the 5 was a holdover from the SAA days.
 
"M-9s are carried with full mag, a round in the chamber, hammer down, safety off."

Remember the Navy pilot shot down over Bosnia and eventually rescued? I've seen him on TV explaining that his 9mm was unloaded until it was time for him to run for the rescue helicopter! Here he is in a war zone, with bad guys looking for him, some very close, and my guns at home are more ready for action? (Don't worry, I've addressed the kids access issue.)
