I find it odd that both mags are repeating the same failure.
Inertia feed from a 1911 type design. If it stumped your local gunsmith you need a new gunsmith.Invited into this one from afar...so...ok.
In 1910, John Browning put a little bitty bump on top of his 1911 magazine follower.
And now you know why.

It's known as "Jumping the Follower" and it occurs when the cartridge rides forward in the magazine almost to the release point...not far enough to let another round come up...but far enough so that when the slide hits it, it gets knocked into the chamber ahead of the slide instead of rising into the extractor from below.and...would you be so kind to explain "inertial feed" it brings stuff to mind but want to be sure what you mean! thanks! edit...oops sorry that wasn't you that posted that!
