Safariland
I ended up going with Safariland.
I got the 5188 paddle for my XD and the 6378 paddle for my M&P.
It doesn't look like they offer one for my PT92 (w/rail).
I emailed support about a couple of their PT92 models (5188 & 568), but they said they won't work with the railed model.
Quality and fit are great. I like the suede lining of the Safariland for added finish protection too.
Both were pretty reasonably priced...$35 & $45, although most of their holsters are quite a bit more.
The 6378 M&P has a locking mechanism you work with your thumb, while the 5188 XD does not. I don't really like the lock, but that's all they had for the M&P.
