The weight of a shotgun takes most of the recoil out of typical rounds, some loads (notably heavyweight turkey rounds) do have a significant recoil, but your average round is nothing that you couldn't fire one handed and easily manage.
Granted the guy in the photo looks like a dumbass, but in reality if you were to hold a pump action shotgun "pistol style" and fire off some birdshot, a trap load, even most waterfowl loads, it wouldn't strain your grip at all.
