All kidding aside, a 1911 will give you the most options for custom parts. Second option would be a Smith&Wesson revolver.

How about that autoeject monstrosity Jim March built out of a .357 Ruger single-action? :) (probably have to remove the smiley face on the ejector housing to make it "goth" enough)
 
To tell you the truth, "Webley" came to mind before anything else but I thought it might be e better idea to get something a lot less maintenance and ammo availability friendly than a top-break revolver. I'd also say the C96 looks like a "Goth" era weapon but I sure as hell hope you don't take a saw/dremel/stick of dynamite to it just for looks. An engraved single action, maybe a matched pair, would fit the bill.

Here's a better one than that other Ruger I mentioned.:

Doc Intrepid, I have the complete Sherlock Holmes at my place, and to my recollection, when Holmes carried, he carried a Colt revolver.

Watson had his Army revolver, which is what it was usually called in the stories, but I am not sure if Conan Doyle ever specified Enfield or Webley.

Edit: +1 on a 1917 in Colt or S&W flavor, unless she's looking for a carry pistol - I have a Colt 1917, and its concealment poses challenges. For a carry pistol, why not a Colt 1903 .32acp? Or a 1908 .380? Those are pushing into early Edwardian, but still should meet Goth/Steampunk criteria.

"Goth" stuff is sorta spikey-like, right? How about a nice "spikey" 1911 from Schuetzen Arms Works? This one's in stainless, but have it Duracoated matte or satin black and it's perfect. And it already has spiders on the grips.

seriously, since its going to be a fashion accessory what type of "goth" is the intended user? thats the only way to determine what would meet the physical appearance requirement.

as a side note, most "victorian goth" simply dress up like the charecters from the movie "sweeney todd". so really the only ting the stylistically would work must come from the victorian era.
 
+1 to the broomhandle suggestion. I think a nice big SAA in a 7 1/2" bbl would look pretty "goth sexy". Or one of those ridiculous encores (doesn't fit caliber criteria, I know)?
