Does anyone know the process? Do you need a handgun permit?

Also is it legal it keep a loaded shotgun on the back seat in plain view?

NC firearms laws

Any individual receiving a firearm in NC, whether purchasing or as a gift, is required to obtain a purchase permit from the sheriff's office in their county of residence unless they have a concealed handgun permit.

Transporting a firearm of any sort in a vehicle is allowed if it is not both concealed AND accessible (unless one has a CHP or is otherwise exempt); it doesn't matter if the gun is loaded or not. As a practical matter, however, I can see little reason to do so and a number of reasons not to, safety concerns being foremost among those. If you want to have a gun in the vehicle, I suggest getting a CHP.

Read "North Carolina Firearms Laws" on the NCDOJ website at http://www.ncdoj.com/getdoc/32344299-a2a7-4ae5-99fd-9018262f64ac/2007-NC-Firearms-gun-Laws.aspx for more complete explanations. Pages 3 and 20 have the information relevant to your questions.

I dont know what county you are in, but you will probably need a "pistol purchase permit" which allows you to purchase a pistol. If you wish to carry it concealed you need a concealed carry permit. Depending on the county it usually isnt muchh of an issue beyond the application and fingerprints. Durham county has its own set of rules. They do require a regestration though, since I dont live there I dont have much of a clue beyond that.

woodsman gave you a good reference. As to the loaded shotgun on the back seat, I see little problem as long as it is in plain view. Pretty commen around here in the country.
