Designing / building an "Inherently quiet HANDGUN"

The STGW57 has a "double shoulder" in the chamber which swages the outside corner of the case slightly as it chambers. This is to cushion the bolt in full auto for more control, but I'll bet it also reduces the bolt clash a bit.

A rubberized or polymer exterior likely helps as well. As far as blast is concerned, just get a silencer. Nothing else works.

on a serious note... I think you 1st need to go past the redesign of the gun, to a redesign of the ammo...

if there was a propellant that didn't offer any report when it burned & the gasses expanded... that might go a long ways towards quieting things... may still require a gun redesign as the properties would likely be different

Can you get to quiet, ie not need hearing protection and NOT risk your hearing.
The answer to that question is "no". Even the quietest 9mm silencer on the market can't get the sound down to safe levels, so how could that be accomplished without a silencer at all?

you need to get it below 135 dB to avoid hearing damage.
No. Any audiologist will tell you that even 134 dB is still way too loud. In fact, even sounds as low as 85 dB can permanently damage your hearing from repeat and prolonged exposure.

Considering that there's no silencer on the market that's truly "hearing safe" (unless you're using unconventional specialty ammo), the OP's goal of making a 9mm "hearing safe" without a silencer simply isn't possible with current technology.

Why not use the .45 , 230 gr loads are subsonic. The answer would be to use a piston to drive the gun and other holes in the action to bleed off gases before they exit the muzzle. Probably wouldn't be too hard to do.

There's not much you can do with the gun to make it quieter, so work with the cartridge. What has been done in the past is a self sealing cartridge. The powder in the cartridge is under a piston, when fired the piston transmits the energy to the bullet and then the piston seals the cartage case, since no gas escape's, there is very little noise.

That's an unusual approach -- and one I had not read about, before. Glad you mentioned it.

Seems as though it would take a very special powder to get the right type of chemical reaction and force wholly within the space of the cartridge. (Seems like that would have to be a very fast burning propellant or a very anemic round.)

Umarex makes really nice co2 pellet pistols and revolvers he would likely enjoy plinking from the porch with. Ammo is cheap too.

The self sealing piston ideal has been done, both by the US and the Russians. The original concept was for use in confined spaces ( read tunnels in Viet Nam )., for the Russkies it was for their Special forces, their Spetsnaz .. There were a number of problems. weight and cost of the ammo, and not quite enough oomp. It never went past a few prototypes and a few test guns., At least I don't think it went into production, no telling what the Russians did.

Another approach

another thing you may or may not be able to consider is modifications to the covered porch to minimize the reflected sound. A thick indoor/outdoor carpet, some acoustic materials on the ceiling and some furniture with heavy padding might help.

I understand the question and how you arrived at it, but I dont see the point of avoiding a tried and true suppressor.
If its quiet without a suppressor it'll be underpowered, so why not just use a 22lr, 22 Short or air rifle/pistol? There are several really nice ones to pick from.

Any successful attempts to create a quiet and powerful cartridge/pistol combination would easily exceed the cost and simplicity of buying a ordinary suppressor.

Your friend who lives in the country, but chooses to shoot from under his porch without hearing protection, I'll quote Forest Gump...
"Stupid is as stupid does"
