My guess is the 21 round magazine protrudes by a inch or so(looking at Magpul’s website pictures). If the pistol is worn strong side, 3-4 o’clock, every time you sit down in a chair or vehicle, the magazine base can hit the back/side of the chair. The constant pressure and daily repetition of contact with it would wear the catch enough over time. 2nd guess is the gun fell directly on the magazine.
