The Win1911 had to be cycled by grasping the knurled portion of the barrel about 6" back from the muzzle, as it didn't have a charging handle on the bolt to avoid infringing on Browning's patents (it's a long- recoil design also). That means in order to load the first and subsequent shells, barrel and bolt go back together, bolt locks back, barrel returns to battery, then the magazine feeds a shell onto the lifter and the bolt slams back into battery also. Lots of people cycled the first round by putting the butt on the ground for leverage, and some of them leaned over the muzzle in the process, others put a hand on the muzzle to push the barrel down. Imagine an AD in that position...
There were some significant mechanical shortcomings to the design as well as the danger it posed to careless operators, and manufacture of the gun was discontinued in 1925.
