What makes a semi-auto "Double-Feed"

I have a gun that was failing to eject. I took the ejector out and cleaned it, and even seemed to fix that problem. However, now I am getting Double feeds- where two cartridges are wedged in the feed ramp, one high, one low. Is this perhaps caused by faulty magazine feed lips? Or weak recoil spring causing battering? I am unsure where to look next and thought I would ask the experts.Thanks for any ideas on how to proceed.
 
It could be the magazine. Also do you slap the magazine in the magazine well like you see on tv. That sometimes will cause it. Also trying to fit an excessive amount of rounds in the magazine will cause a double feed.
 
The most important question is what kind of pistol do you have.

Double feeds can be more than just a mag problem.

What are you shooting and what kind of ammo?
