Are these the 2 forms needed for C&R application?

F 7CR - Application for License (Collector of Curios and Relics) Under 18 U.S.C. Chapter 44, Firearms

F 5330.20 - Certificate of Compliance With 18 U.S.C. 922(g)(5)(B)

Anything else?

Just 7CR. That's all I sent in (still waiting for my C&R though).

I guess 5530.20 used to be required, but is now included in the new 7CR so as I understand it that one isn't required.

how do you get a ATF F 7CR form?

either call the ATF and get them to send you some, or use this: http://www.calguns.net/CR/CRFFLForm7CR.pdf (its got the interactive text boxes, so you can fill it all in on your computer, save, and print)
Remember to print on the front and back side of the piece of paper..
