Staked-on front sights

I understand that staked-on replacement front sights for a Colt Government Model can sometimes come loose. Is this problem inevitable with staked-on sights, or is it a result of improper installation?
 
AT, this is from improper installation. I'm not going to say it has never happened but I will say that I don't ever recall hearing about a front sight coming loose that was installed properly. George
 
I agree with George, and just want to add a note that the larger and heavier sights have more tendency to come loose than lighter sights. (The small GI sights, for example, are all staked in - except for the original Springfield Armory guns - and they don't come loose.)

Many smiths will silver solder the heavy ones in place.
