S&W Mod 10-8 4" not indexing

I recently cleaned my S&W Mod 10-8 (4" hvy bbl) and I removed a lot of built up crud behind the hand - 10 years of weekly shooting. Now it appears as if the hand does not protrude forward enough to catch the ratchet face on the cylinder (extractor). Is there a simple solution to this problem. Does a new over sized hand need to be fitted? Any help would be appreciated. End shake has been corrected.
 
Is it possible that you reassembled it incorrectly? I would be suprised that a good cleaning would make functioning pistol not work.
 
Maybe you put something together incorrectly. I doubt the smegma was making the gun function properly
 
As above, you either failed to reassemble it properly, or some hard fouling displaced and fell into the action and is jamming it.

Most likely suspect if you disassembled it is that you failed to get the spring on the hand correctly located, or it slipped loose and you didn't notice.
 
If you removed the hand from the trigger, chances are that you disengaged the hand spring, which is inside the trigger and bears on the little forward pin in the hand. If that is the case, take the hand and trigger out again and use a small screwdriver to lift the spring so the hand can be put in right. Don't reassemble the gun until you are sure the hand has the proper tension.
