Pearce finger ext popped off G27 mag!?

Anyone had this happen? I have a G27. One of my mags has the Pearce +1 finger extention. When I first got it, it was extraordinarily difficult to get ten rounds in it. VERY difficult. I was told it would 'loosen up' Anyway, I noticed this weekend that one side of the floorplate had popped off the flange on the bottom of the mag. I put it back on, and took it to the range. Not very reassuring.
 
Are you sure it was put on correctly??? Have you dropped it on cement when partly loaded??? They seem very secure on mine.
 
Perhaps that particular extension was molded improperly?

I have 6 Pearce floorplates on my G26 mags and none of them have ever shown signs of loosening.
