paperwork question for SBR

I am currently building a SBR and the paperwork is confusing the heck out of me.

Short of paying the $200, what else is required and in what order?

Do I need to get fingerprinted, have the CLEO sign off on it - and what else.

if the rifle has a collapsible stock, how do I measure to overall length that the ATF wants in the paperwork?

please help.
 
You need to send duplicate fingerprint cards, passport photos and get CLEO sign-off just like with a form 4. And don't forget your 18 U.S.C. 922(g)(5)(B) compliance form either!

(You'll also need to engrave your name, city and state on the receiver or barrel.)

As for OAL, measure the rifle extended to its maximum length. You can also specify a range of length, in which case you'd also measure it collapsed.

two final questions, without the upper, how do I measure the overall length of weapon? Do I extend the stock, measure and then add the appropriate barrel length?

and lastly, in section 4.d it simply says "Model" and I can't find any info pertaining to this in the instruction sheet they sent with the forms.
