You need a square forked tool to engage the nut on the LH side, and a pointed punch to stake the new screw to the new nut. The punch is easy to come by; the fork, not necessarily so. I have, however, heard of people making their own fork by carefully filing down a flathead screwdriver.
Yes, the rear blade is broken. I have already ordered a new one from S&W. I wasn't going to pass on it over a minor repair.
