Milt Sparks Versa-Max or Mitch Rosen ARG (Ayoob Rear Guard). Superior quality, thinner on the pistol because the belt loops are not over the pistol but front/rear (or just to the rear, in the case of the ARG).
 
Ted Blocker LFI

I have a Ted Blocker made LFI rig for a full size Beretta 92FS that I have put a good amount of wear in and it has worked very well.
It conceals the handgun very well and is about as comfortable as any concealment holster I have worn. It was the first what I refer to as serious concealment holster I purchased several years ago and I still really like it. I prefer the thumb break model as I have seen and realized, the real world need for the added security in every day wear.
The LFI includes a belt with the inside lined with velcro and the holster and single mag pouch have velcro on a sturdy tab that mates with the belt. It makes for a holster that stays where you put it and provides about an extra inch of ride up on your shirt or jacket.
