Does anyone have a writable ATF 5330.20

I'm trying to find a certificate of compliance to send in with a Form 1, but every one I can find (including the ATF version) can't be typed into. Anyone here have a version that can be edited or typed into?

Thank you David! I noticed that the newest revision is April 2006, that's the 2005 form... do you think there's any problem using it?

They're pretty similar, just afraid to send the "old" one and run into problems. And haven't really paid attention when dealers have filled them out for me when submitting F4's.
