Definition of FTE

Is it a classic FTE when...

After you fire a shot, you realize, "hey, the slide hasn't closed all the way," and then note that the old casing and the new round are kinda battling for space in there... Not a stovepipe (the case is not vertical)...just two cases trying to play leapfrog...

And, whether this fun little scenario is an FTE or not, what do you do about it? Is it a matter of adjusting the extractor and ejector?
 
If the fired casing has been removed out of the chamber but hasn't been flung out, than it would appear the extractor has done its job while the ejector has not.

There are possibilities however, such as the extractor disengaging the casing as soon as the casing has been fully removed from the chamber.

Though the scenario you describe makes it seem more of an ejector malfunction (especially if it happens quite frequently).
 
It may be a poor job of cleaning.

I had a FTE problem with my SIG 229.

I had shot a lot of Wolf ammo, which is a dirty ammo.
Looking down into the mag well with the slide locked back the extractor looked clean.

When I removed the slide from the frame and looked a the extractor in day light I could see a ramp of dirt below the extractor.

Cleaned that up and no more trouble.
