Colt hammer cam?

Is it possible for the little cam the moves the bolt up and down to be worn so the bolt slips off?

Well, its actually supposed to slip off. At the right time of course.

But it does not go back on, it locks the gun up?

It goes back on when the hammer is down so it can start the cycle over again.

In the pettifogger articles, read with particular interest the section on the shaping of the bolt leg which rides the hammer cam.

IIRC correctly, on some hammers it is possible for the cam to move and get out of position. I don't know how frequently that happens but you may want to check the photo of the cam in the article as compared with the cam on your hammer.
