CCW paperwork from FL questions

I just sent off for a packet for a CCW from FL. I was looking online and a bit confused on some questions,

it says

and the

???
Also where is the box to check to apply for a extra CCW permit incase of lost?

You need to take the paperwork to a notary to have them fill in that part. You can find ones close to you in the phone book or online.

For a lost license, you don't need to re-submit the entire form, just a notarized letter saying that it was lost. From page 7:

"In the event that a concealed weapon or firearm license is lost or destroyed, the license shall be automatically invalid, and the person to whom the same was issued may, upon payment of $15 to the Department of Agriculture and Consumer Services, obtain a duplicate, or substitute thereof, upon furnishing a notarized statement to the Department of Agriculture and Consumer Services that such license has been lost or destroyed."
 
A notary Public must witness your signature and counter sign the application.
You must produce a valid ID to the Notary unless you are personally known to them.
 
Can a notary be a cop from my local PD when I go to submit my prints for my own state CCW? or the guy doing my prints?

I don't even know what a notary is. Never used one.


*edit* I just looked it up, theres a guy right down the road that does this service along with other stuff like mailing/stamps etc..

Any idea what a fee would be? $5?

3rd edit* I notice UPS does this also.
 
Normally, a Notary is a person bearing a commission from his State of residency. He'll have a seal and usually will embosse the document.

Suggest you use your bank for the thing, you might have some difficulty using a LEO unless the DOA say's different.
 
Not in FL. I want a FL CCW permit + my home state, because FL does more states than my home one.
 
The reason I ask is because you can make an appointment at your local Florida Dept of Agriculture office where they will do everything for you at no extra charge.
Fill out the app on computer, they'll take your photo, do your fingerprints and notarize the whole shebang at no extra cost. And you'll get your license in the mail in 2 or 3 weeks instead of the 2 or 3 months it takes if you mail it in.
 
How do you do it on the computer/send fingerpritns in?
Also how do you send it in via electronic?

I can send the finger prints in on cards, but also my local PD can do it electonicly..? They just fax it?
 
You cannot do it electronically if you are not in Florida. Take the FL card to your local PD and ask them to do the fingerprints. Be SURE to follow the instructions exactly. Florida requires the officer taking your prints to sign the card on the BACK side. When I last renewed mine, the officer signed on the front and FL rejected the card. They sent me a new one and I had to do it over.

It is unlikely that a police officer will be a notary public. Ask at a bank, or perhaps at your city hall. Do NOT sign anything until you are in front of the notary public. The notary is certifying that he/she saw you sign the document. If you show up with a paper that you signed at home, they cannot certify it.

Its funny how Florida issues non resident permits, but doesn't honor non resident permits from other states.
