My BHP has recently developed a problem:

The hammer follows the slide when the slide returns to battery.

Is this a sear issue or a sear spring issue?

How would I replace the rear spring and/or sear?

It could be either/both and simply bending the sear spring would be fast/free/easy way to learn more -- if not actually "fix."
 
Sevens has said it all, the sear or spring are causing the issue. If you cannot disassemble the pistol take to a competent gunsmith for replacement although not a difficult project. I do not normally bend springs but replace as they are available and cheap.
 
To be clear (in hindsight... I wasn't) my suggestion to bend the sprimg was simply a cheap/fast/easy way to see if the spring is the likely culprit. Bending it, giving it more "spring" may get the pistol acting normal again. If so... and the spring is the problem, bending most certainly did NOT fix the spring, but temporarily it may indicate that the sear is fine.

The spring likely needs replaced either way and if you bend it radically to get what you want out of it... then it REALLY needs replaced! ;)
