I've filled out a few of these the past couple years, and for the most part the wording is the same even though the forms seem to morph somewhat.
However at WalMart last week I noticed something different. When asked if you had ever been convicted of a felony, it also mentioned if the felony was something you could go to jail for a year, or if you were convicted of a felony...given a year's sentence, then reduced (or thereabouts).
The form was dated 2005 at the bottom. I had never seen one like that.
EDIT: They also asked for a daytime phone number. Can they do that? What is the purpose?
