my 38's chamber/cylinder will only spin after firing if it is aimed slightly downward. i figure something is loose or not clicking right. if i aim it slightly downward then no problem. the cylinder spins and i can empty all six rounds. but if im aiming at a target that is even with my head it will fire the first round then refuse to spin. any ideas?

It may be a broken, missing, or misaligned hand spring. So the hand isn't resetting to engage the cylinder's ratchets. But if the revolver is pointed downward the hand falls by gravity and resets properly.

If so, you're going to need to open-up the revolver and check the spring - it's not a big deal to replace or fix the spring.

BTW what make of revolver is this?

i got it from my grandfather. im not real familiar with handguns so please excuse my lack of knowledge:) it says rohm 38 special on the side. its not a snubnose. ive heard they are mediocre guns at best but until today it fired great and since it belonged to my deceased granddad it has sentimental value. when i get a little extra buck im going to invest in something better and put this one away in a safe place.

Is this when the yoke is open? If so, its possible the cylinder, or the rims of the casings, is coming in contact with the "retaining stud"(lack of a better word). All my Rossi's and S&W's do that.

Single action or double? Does everything else (hammer, trigger) move but the cylinder doesn't or is it completely locked up when not pointed down?

Rohm Geschellschaft= RG
Parts are hard to come by, a gunsmith will want more to fix it than the gun is worth, and if you take it apart you may never get it back together again. I would recommend you retire the gun.
