1911 shooting more than bullets

I was at the indoor range the other day adnd well.... my Charles Daly 1911 shot more than bullets downrange. also expelled the recoil spring and the Cap at the end of the recoil spring that contacts the barrel bushing. why?

I am thinking that the last time I cleaned I didn't assembly it right. any other ideas? it happened on the 3rd round of the day anyway i will be more careful when I reassembly me 1911 from now on
 
I have heard of some bushings breaking and letting the recoil spring and plug fly down range. These bushings are usually thin and are supposedly cast metal. A good quality bushing properly fitted will not break and can improve accuracy.
