1911 Grip Safety Doesn't work

My 1911 will allow the hammer to fall without the grip safety being depressed. Is this something I could have caused when assembling, or likely a bad part? It works normally otherwise.

Several things can cause this condition:
1- The sear spring has been bent to try to reduce trigger pull weight and the little finger that pushes the grip safety into position was also bent and not pushing it far enough.
2- A new trigger or a new grip safety was installed and not fitted, not allowing the nose of the grip safety to block the back of the hammer.
3- The nose of the grip safety was altered for some reason and does not protrude far enough to block trigger travel.

Whatever the issue, it should be fixed. The grip safety is one of the 3 safeties on the 1911. It is supposed to keep the gun from firing in the event the trigger is depressed without holding the grip. It is possibly not unsafe in this condition, but it would be better if it were fixed. If you are not knowledgeable in 1911 operation, take it to a gunsmith.

I've had it apart doing things like ambi safetys, cleaning and stuff that has required me to remove the grip safety, so I suspect I'm the culprit. I'll check those items scorch mentioned. I wonder if having one of those spring ends under or on top of something it shouldn't be could cause it. This has been my project gun and it's taught me a lot. This will be one more thing.
Harry, in the past you've advocated relieving the chamber of match barrels. This one (Kart) has a feature I'd like to cure and that is that when the chamber gets very dirty the gun sometimes doesn't go fully into battery, without a shove. Is that the relieving you talked about, to prevent this embarrasing problem? If so, does it affect the "match" chamber benefits, or perhaps is the match chamber just for "matches" and not for duty?
